A postmortem (or post-incident review) is a written analysis after an incident: the timeline, the root cause, the impact, and concrete action items to prevent a repeat.
The most effective postmortems are blameless — they focus on systems and processes rather than individuals, which encourages honesty and real learning. Done well, they steadily reduce both the frequency and the impact of future incidents.
